
River Valley Nursing Home takes pride in being adequately staffed with educated professionals and experienced staff. It is our goal to provide a homelike environment to our residents while gently balancing their medical and emotional needs. Our staff continues to train and educate themselves on the newest and most effective methods for the best resident care offered.
Ken Urlage – Administrator
Ken Urlage grew up in skilled nursing facilities. His family has been in the business for years. Ken is one of the most respected and called upon administrators in the State of Kentucky. He is active in the KAHCF (The Kentucky Association of Health Care Facilities). He is a past recipient of the prestigious “Administrator of the Year” award. He has been in the long term care business since 1964. Ken is married and has 3 children and two grandchildren. Ken’s hobbies are golf, jogging, and Wally Ball . Ken is a former Marine and served his country proudly in the Vietnam War.

Lori Angell– Director of Nursing
Lori started her career as Director of Nursing in October 2012. She began River Valley Nursing Home as an LPN while working her way through Galen School of Nursing. She graduated a registered nurse and soon advanced to River Valley's Infection Control & Quality Assurance Manager in 2011. After 1 1/2 years, she was promoted to Director of Nursing. Lori was born and raised in Pendleton County. She is married and has two children. She is an active member of Trinity Southern Baptist Church. She enjoys sports and spending quality time with her family.

Mary Coleman - Social Service and Admissions Director
Mary Coleman is a lifelong resident of Pendleton Co. She is a graduate of NKU and has been a social worker for 15 years. She has 7 years experience in long term care. She knows from personal experience how difficult it is placing your loved one in long term care but has seen firsthand how the staff love and care for them just like family. She finds the opportunity very rewarding to be able to build personal relationships with our residents.
Mitzi Yelton - Business Office Manager
Mitzi started her career with River Valley in November 2011. She has attended college at Morehead State University and Trident Technical College located in Charleston, SC. Mitzi has 9 years experience in billing and office experience. Mitzi has two children and enjoys reading, making shawls, and spending time with her children. She is a member of Butler Baptist Church and the Falmouth Festival Committee.
Robin Mulloy – Activities Director

Robin has been the Activities Director since November, 2012. Before that, she was the Social Service Director for 15 years. She was awarded the prestigious Social Service Admissions Director of the Year award presented by the KAHCF, The Kentucky Association of Health Care Facilities. Robin has been a resident of Pendleton County all her life. She has been involved with long term care services since 1980. She finds joy in building friendships and personal relationships with our residents, their families and our staff members.
Carol Hurst – Dietary Manager
Carol has been our Dietary Manager since 1982. She has won the Honorable Mention award for Dietary Services from the KAHCF, The Kentucky Association of Health Care Facilities.Carol has lived in Pendleton Co. all of her life. She is married and has one son, one daughter and one grand-daughter. Carol enjoys reading, cooking, quilting and collecting cook books and dolls.
Rebecca Robbins - Restorative
Rebecca started at River Valley Nursing Home in 1995.She started her career as a CNA. She moved to LPN and advanced to Director of Restorative. She has maintained her position with 7 years experience. Rebecca is married and enjoys reading and crocheting.
Crystal Sorrell-Infection and Quality Control

Crystal began her career with River Valley Nursing Home in February 2009. She is a graduate of Northern Ky. University and has a Bachelor Degree in Nursing & Science. She started as a Registered Nurse and has advanced her position to Infection Control & Quality Assurance Manager. She is a lifelong resident of Pendleton County. She is married and has two children. She enjoys reading, decorating and creating flower arrangements.
Reva Combs - MDS Coordinator
Reva started her career with River Valley in 2011.She graduated from Northern Ky. University with a Bachelor degree in Science & Nursing. She started as a Registered Nurse and has advanced to MDS Coordinator. Reva was born and raised in Newport, Ky. She is married with three children and enjoys making hair bows and reading.
Aletha Smith - Unit Manager
Aletha started her career at River Valley in 1992 as a CNA. She proceeded with her education and advanced to LPN, followed by Registered Nurse as of 2012. She is a grauduate from Maysville Community College. She is currently employed as a Unit Manager. She has two children and three grandchildren. She enjoys reading and attending Gumlick Baptist regularly.
Vickie Carr - Administrative Assistant
Vickie started her career with Licking Valley Oil in 1997 and transferred to River Valley Nursing Home in December 2006. She has maintained her position as administrative assistant and has 15 years experience.Vickie is married, has two children and one grandchild. She enjoys reading, loves music and is actively involved at Blanket Creek Church.
Nancy Barger – Housekeeping & Laundry Manager
Nancy has been involved with long term care since 1993. She has lived in Pendleton Co. all of her life. She has been the manager of Housekeeping & Laundry since 2006. She is married and has one son, two daughters and seven grand-children. Her hobbies include crocheting, reading and gardening.
Brian Thomas - Director of Maintenance
Brian has over 30 years experience in the maintenance field. In 1985, he joined the US Navy. He served 20 years between active duty and in the Reserves. Brian retired from the armed forces in December 2005. Brian has worked in the long term care maintenance field since 2000.Brian received the AOPHA award in 2004. He started River Valley Nursing Home in October 2011. Brian is married and has two children and one grandchild.Brian likes to hunt, fish, boat and enjoys the outdoors.
